About the Support at Home Program

The Support at Home Program brings together some of the existing in-home aged care programs into one model that focuses on what you need to stay safe and well at home.

It provides a clearer way to access and manage services, while giving you more control over how your support is delivered.

Under the program, you can access help with daily tasks, personal care, health and clinical services, transport, social connection, and more all from one trusted provider.

 

The Support at Home Program

On 1st November 2025, the Support at Home Program has replaced the Home Care Packages and Short-Term Restorative Care programs.

What this means for you:

  • One entry point for in-home services through My Aged Care
  • Funding matched to your needs through a new assessment and classification system.
  • Services that adjust as needs change.
  • Access to additional supports including equipment, technology, and home modifications
  • Clearer, simpler pricing
  • Two short-term pathways: Restorative Care and End-of-Life

How to Apply for the Support at Home Program

Getting started is straightforward, and we’ll help you through each step.

  1. Contact My Aged Care – Call 1800 200 422 or visit myagedcare.gov.au to register.
  2. Have Your Assessment – An assessor will visit you at home to discuss your needs and goals.
  3. Nominate Coolibah Care – Choose us as your preferred provider so we can help design your care plan.
  4. Create Your Plan – Together we’ll shape a plan that feels right for your life.
  5. Begin Your Services – Our local team starts delivering care with consistency and compassion.
