Coolibah Care – CHSP Providers
If you need support with just a few tasks, such as cleaning, then you may be eligible for CHSP services. The programme is designed to aid those who need a low level of support to continue living independently and safety at home.
Domestic Assistance
We are CHSP providers offering a variety of domestic services covered by the CHSP, which can help you with various household tasks.
These CHSP services include:
- Cleaning: Keeping your home clean and tidy is essential for your comfort and safety. Our team can assist with regular cleaning tasks, including vacuuming, dusting, mopping, and general household maintenance, ensuring your living space remains a healthy and welcoming environment.
- Ironing: Ironing can be a time-consuming and physically demanding task. We offer assistance with ironing your clothes, helping you maintain a neat and presentable wardrobe with minimal effort.
- Laundry: Managing laundry can become challenging over time. We provide laundry services, including washing, drying, folding, and organising your clothes, so you can enjoy fresh and clean garments without the hassle.
- Unaccompanied grocery shopping: If you find it difficult to go grocery shopping on your own, we can help by taking care of your shopping for you. Our service ensures that your pantry is stocked with the essentials you need, without the strain of navigating stores or carrying heavy bags.
- Meal preparation: Proper nutrition is vital for maintaining your health and well-being. Our team can assist with meal preparation, helping you plan, prepare, and cook meals that meet your dietary needs and preferences, ensuring you enjoy delicious and nutritious food every day.

Coolibah is a CHSP provider that offers in-home social support and accompanied outings such as shopping.
We’ll get you out and about so you don’t miss that important appointment, a social event, or that special coffee date with friends.

Getting started if you don’t have a Home Care Package…
Step 1 : Call My Aged Care on 1800 200 442 or go online at www.myagedcare.gov.au
Request an assessment.You’ll be asked a series of questions to assess your eligibility. Following this a meeting will be booked with the Aged Care Assessment Team (ACAT).
Step 2 : An ACAT assessor will complete an ACAT Assessment.
You’ll be asked more detailed questions to determine the level of care you need, your eligibility and also your priority.
There are four levels of support ranging from Basic Care Needs (Level 1) to High Level Care Needs (Level 4).
Step 3 : You’ll receive a letter from My Aged Care with the outcome of your assessment.
If you are eligible for support, you’ll be placed on a national waitlist.
Waiting times vary but may take from three (3) to twelve (12) months to become available and is dependent on the level of the Home Care Package you are to receive.
Step 4 : You’ll be assigned a Home Care Package
You’ll receive a confirmation letter assigning a Home Care Package and advice on when it will commence.This gives you time to choose your preferred Home Care service provider.

Step 5 : Choose your Home Care Package service provider
Once you’ve chosen who to partner with for your Home Care services, give them the unique Referral Code on your letter from My Aged Care. They can then accept your package and get you started.
What is the difference between CHSP and Home Care Packages?
The main difference between the Commonwealth Home Support Programme (CHSP) and Home Care Packages is the level of care that is required to live independently.
How do I switch to Coolibah Care?
If you’re not happy with your current service provider, or you want to get better value, you can switch at any time.
You’re not locked into the Home Care Agreement.

Can I access services privately?

Can I get both CHSP services and HCP services at the same time?
Generally, this is not allowed to ensure that the services can be provided to as many people as possible.

What is the process to apply for residential care?
To apply for residential care at Coolibah Care an application form needs to be completed, accompanied by supporting documentation such as an ACAT assessment. Click here for more information or get in touch with us.
What services am I eligible for through the Support at Home program?
The services you receive depend on your assessed needs and the supports you have been approved for. To learn more, contact our Home & Community Team or visit our Support at Home page.
What respite services do you offer?
We offer a range of services including day, low care / cottage respite and residential respite for a higher level of care. Referral codes are required for these services. Reach to our Respite Coordinator or visitor our Respite Care Page to find out more.
Do you have independent living unit available?
Contact us to find out about our vacancies in our aged care independent living village. You can apply online and we do take waitlists if there is no immediate vacancy at our Mandurah retirement village.
Voluntary Assisted Dying (VAD)
Voluntary assisted dying is designed to offer eligible Western Australians an additional option for end-of-life decisions within the context of high-quality, patient-centered end-of-life care. Coolibah Care adheres to the Western Australian Voluntary Assisted Dying Guidelines to ensure compliance with the Voluntary Assisted Dying Act of 2019. Coolibah Care does not engage the services of medical practitioners or nurse practitioners to facilitate residents’ access to voluntary assisted dying. Consistent with legislative requirements, Coolibah Care will not impede a resident’s right to access voluntary assisted dying, nor negatively impact a resident’s dignity or right to choose this option. Coolibah staff may be approached in relation to voluntary assisted dying and will escalate the matter to a senior member of the clinical team to support access to appropriate information and resources relating to the VAD process.
