If you are still working out what your loved one needs, read on. This page explains the specialised care available at Coolibah Care and how it adapts as needs change.

Care that adapts as needs change

Needs rarely stay the same, and care at Coolibah Care is designed around that reality. Our residential aged care home offers a range of care levels, from medium through to high care, along with respite stays and specialised dementia and palliative care.

We support ageing in place wherever possible, which means that as a resident’s needs increase, their care increases with them. For families, this brings real peace of mind: the person you love can stay in a familiar place, with a team who already knows them well.

Dementia care at Coolibah Care

Coolibah Care has two secure areas purpose-designed for people living with dementia. Each resident has a private room with an ensuite that can be personalised with photos, mementos and familiar items, helping them feel connected to their surroundings and their own story.

Care is delivered by a team trained in dementia-specific practice, with consistent routines and an environment where residents can move about freely and safely. Families are warmly welcomed and encouraged to stay closely involved, and our Coolibah Dementia Support Series offers honest, gentle guidance for the questions families ask along the way.

Palliative care at Coolibah Care

Palliative care at Coolibah Care focuses on comfort, dignity and quality of life for residents approaching the end of life, and on supporting the people who love them. Our aged specialised palliative care team works alongside each resident’s GP and family to manage pain and symptoms, honour personal wishes and provide gentle, respectful care through the last stage of life.

Families are supported too, with clear communication, space to be present and guidance through decisions that can feel overwhelming. No one walks this road alone at Coolibah Care.

Common questions about specialised aged care

What specialised care does Coolibah Care offer?

Coolibah Care offers dedicated dementia care in two secure, purpose-designed areas, palliative care supported by an aged specialised palliative care team, and a range of care levels with ageing in place. Respite stays are also available, including dementia-specific respite.

Can my loved one stay at Coolibah Care if their needs increase?

Yes, wherever possible. Coolibah Care supports ageing in place, which means care increases as a resident’s needs change. Most residents can remain in a familiar environment with a team who knows them, rather than moving to a new facility.

Do I need an assessment before accessing specialised care?

Yes. Residential aged care, including dementia and palliative care, requires an aged care assessment arranged through My Aged Care. Once the assessment is complete, an application form is submitted with the supporting documentation, and our team can guide you through each step.

How do I know which type of specialised care is right for my loved one?

Start with a conversation. Every situation is different, and the right care depends on your loved one’s diagnosis, current needs and what matters most to them. Our team is happy to talk through your circumstances, with no obligation, and point you in the right direction.
